1. Parent/Guardian Expense Reimbursement Form:
• Private School Tuition & Fees: Reimbursement
• Specialized Services: Reimbursement (Tutoring, therapies etc.)-* recommended to first fill out a pre- authorization form.

2. Direct Pay to Approved Service Provider Form
- Private School Tuition & Fees: Direct Payment
- Specialized Services- (Tutoring, therapies etc.):Direct Payment
•Pre-authorization for a non-approved vendor, educational materials, and curriculum
