In consideration for being allowed to participate in the Teach Florida Mission to Tallahassee, including a charter flight to and from Tallahassee, ground transportation and all other segments of such event (collectively the “Mission”), on or about April 2, 2025 the undersigned acknowledges and agrees that:

Teach Florida, a program of the Union of Orthodox Jewish Congregations of America (the “Orthodox Union’) has made certain arrangements for participation in the Mission, including arrangements for my transportation in a general aviation aircraft through Exclusive Charter Service, Inc. Worldwide Aircraft and Yacht Charter I acknowledge and understand that there are certain risks that may occur from participating in the Mission,including, but not limited to, those associated with flight in airplanes, including but not limited to injury, illness or, in extreme cases, permanent trauma or death;
Being fully aware, of such risks, I voluntarily, without force or coercion, knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Releasees set forth below or others, and assume full responsibility for my participation. In return for the opportunity to participate in this Mission, I sign this waiver and release freely and voluntarily. I acknowledge that I have read this agreement, understand it, and that no oral representations, statements, or inducements, have been made to me apart from what is contained in this written agreement; and further, that I am at least eighteen (18) years of age and fully competent;
I understand that the payments I may make in order to participate in the Mission are non- refundable under all circumstances and that the organizers of the Mission are relying on such funds to arrange for air charter, ground transportation and other items associated with the Mission and I waive my right against Teach Florida and the Orthodox Union to seek any or all of the funds I may provide to them in connection with our attendance at the Mission;
I agree to comply with the terms and conditions for participation, including, without limitation, rules of the airline carrier and charter company through which the Mission will be effected, whether explicitly communicated to me orally, in writing or otherwise, posted on the airplane or other facilities utilized during the course of the Mission, or customary rules of travel of which a reasonable person would be aware;
I will comply with all applicable COVID regulations and guidelines of the Centers for Disease Control, other applicable authorities, and the airline carrier and other facilities utilized during the course of the Mission;
I will alert appropriate personnel of hazardous or dangerous conditions that I observe, as reasonable under the circumstances; and
In consideration for the opportunity to participate in this Mission and the associated services being provided to me, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby willingly and completely waive, release, and discharge Teach Florida and the Orthodox Union, and any of their respective chapters, affiliates officers, directors, trustees, affiliates, agents, employees, independent contractors, consultants, advisors, vendors, vliunteers, licensees, assigns, successors and (collectively “Releasees” and each a “Releasee”) from and against any and all claims, losses, damages, demands and liability or responsibility, including, but not limited to, economic damages, non-economic damages, special damages, incidental and consequential damages, and property damage, personal injury, and death, which directly or indirectly arise out of the Mission or the delay, inability, cancellation, or failure to complete the Mission. IN DOING SO, I AM RELEASING EACH OF THE RELEASEES FROM ANY AND ALL CLAIMS OR DEMANDS OF ANY TYPE WHICH MAY ARISE, EITHER DIRECTLY OUT OF MY ATTENDANCE AT THE MISSION, INCLUDING TRANSPORTATION BY AIR, EVEN FOR ANY DAMAGES OR LOSSES CAUSED BY THE NEGLIGENCE OF ANY RELEASEE, TO THE FULLEST EXTENT PERMITTED BY LAW.
Indemnification and Hold Harmless. I agree to indemnify and hold harmless the Releasees from any and all claims, actions, suits, procedures, damages, losses and liabilities (“losses”) – including attorneys’ fees and costs – brought against the Releasees, arising out of the Mission, whether such losses are caused by my negligence or the negligence of any third party.
This agreement is to be governed by and interpreted by the laws of the State of Florida.

ORTHODOX UNION MEDIA RELEASE*

From time to time, The Union of Orthodox Jewish Congregations of America, and its programs, employees and agents (together, the “Orthodox Union”) take photographs and videos of individuals, including minors, who attend or participate in Orthodox Union events, programs, and other offerings. The Orthodox Union engages in these activities for purposes relating to the advancement of the religious, spiritual, charitable, and educational goals and objectives of the Orthodox Union, including but not limited to the sharing of photos/videos in newsletters, on our blogs, and on our website and social media pages for the purpose of keeping our members and others informed of the Orthodox Union’s events and other activities. Please complete this form to give us permission to take photos/videos of you and/or your children when you participate in Orthodox Union events or programs and to use those photos/videos in our printed and online publicity:

I hereby authorize the Orthodox Union to take photographs and/or video of me (or my children) when I (or my children) participate at Orthodox Union events or programs. I grant the Orthodox Union full rights to use and share the images resulting from the photographs and/or videos and any reproductions thereof (the “Recording”), in whole or in part, along with my (or my children’s) name, biographical information and quotations for any purpose including fundraising, publicity, advertising, or on social media, all without further obligation or compensation. I hereby waive any right of review or approval regarding the Recording and the Orthodox Union shall have the right to edit or delete the Recording in its sole discretion. I understand that all rights in and to the Recording shall be the Orthodox Union’s sole and absolute property. I understand that if I access the Orthodox Union’s services through a third party platform, product, service, or application (a “Third Party Service”), content and related information generated while accessing OU’s services in this manner may be monitored, captured, and or recorded by the third parties that provide such Third Party Services. I consent to the use and sharing of personal information by any such third party as described in the privacy policy available through the Third Party Service. I release and forever discharge the Orthodox Union from all causes of action, suits, or liability of any kind arising from or in connection with the use or publication of any Recording of me or my children made in connection with my participation in Orthodox Union events. I waive any right of privacy associated with the Recording and I waive any right to compensation related to the use of the Recording.
I HAVE READ THIS AGREEMENT AND AM FULLY AWARE OF ITS CONTENTS, INCLUDING THE RELEASE THAT I HAVE AGREED TO HEREIN.
I warrant that I am at least eighteen years of age (or, if not, that my parent or legal guardian has agreed to the foregoing and signed below) and that I have the full and unrestricted authority to enter into a contract for myself (or, if not, that my parent or legal guardian has agreed to the foregoing and signed below).
If any provision of this agreement is held to be unenforceable, the remainder of it shall continue to be in full force and effect.
This waiver shall remain in effect until such time as I advise the OU otherwise in writing.
